Answers: Nope. Get or print a new form. It's very unprofessional.
NO NO NO it is importantly unprofessional as well as sloppy and the first impression the guy/gal who read it will throw it in the trash- I know I would! as it shows how your mind works-Get another form, and before you mess it up do a rough draft on the one you own already messed up , then fill the investigational form neatly
It may be OK on a form that they supply, especially if you don't own another blank one. Definitely not acceptable on a resume though.
